Token is missing from the report link

As per the sample shown below the token is missing from report link URL, so the end user gets taken to the login page.
This system was upgraded from version 3 to eFa-4.0.4 and everything appeared to restore okay.
I have not noticed any other issues at all.
I have spent a bit of time looking into the issue, but I need to be pointed in the right direction. When is the token added to the link? Where do I look?
