An email that arrives to EFA is not forwarded to all recipients in exchange. Just One

HI,
Users are complaining that are not receiving emails where they where CC.
I chequed email listing in EFA and the delibered emails in Exchange and when forwarded by EFA , mails only arrives to one or other recipients. Not all of them.
To be clear,
USER A@external.com send an email to B@mydomain.com and C@mydomain.com .... in some cases only B receives the email, in other cases only C receives the email, never both of them.
Any workaround with this _?
