SMTP relay/Smarthost problem

Sorry my English.
We use XEAMS for spam filtering (inbound and outbound messages too), but we have to change to other free alternativ. I have tried EFA, my problem is, if is set our ISP-s SMTP relay for outgoing messages, the ESA doesnt send e-mail (no error messages in log, it looks like the massage sent to the right relay). If I change SMTP relay to the ip address of XEAMS (local IP), XEAMS forwards messages to ISP-s relay and the mail is arrive.
I can ping and telnet to the remote relay from EFA.

PS:
I tried to change the hostname to mail.mydomain. In this case i can send messages to outside world, but the incoming mails doesnt come in. I have got this error message in the outside sender's account: "mail for [x.y.z.u (private ip of the local mail server)] loops back to myself"

This not only spam problem, i tried to sent mail to my gmail account and in the spam section is no mail too. Not My address changed, only de hostaname of the EFA server. If the hostname same as my mail server (outside the ip address is same, it is NATted) I can send mail, but I cannot receive from outside. when the hostname different like the mail server, the mails come from outside but I cannot send mail. With XEAMs proxy, there was no problem with different local hostname. The hostnames are only in local USE. Our MX record is the Outside IP address what the router get from our ISP. The mail server and the filter is behind the router and NATed
