Weird behaviour when releasing emails

One of my clients received 2 emails which got caught as SPAM with SPAM > 5
I released both but only 1 got whitelisted. Any ideas why the first released email simply passed and only the second was marked green as whitelisted? Both times I simply ticked the "release" box and clicked submit.
