MailScanner Version - 5.4.2

Today we noticed that some of our custom rules in SpamAssassin were not working properly. When we ran a test from shell, we noticed that our SA rules were working properly, but it was not being applied to client mail going through the filter.
These rules were working properly in the past, but don't appear to be working anymore.
I was looking through the forums, and found this thread: viewtopic.php?f=19&t=4702 which seems to be similar as when SA is tested from the command line it is working properly, but it is not affecting mail flowing through the system. Are these issues related?

That thread is specific to the RelayCountries plugin in which you need to add a header using SpamAssassin. The problem there is that when MailScanner calls SpamAssassin it doesn't capture the header.
