Hmm...there is clearly a recipient address in the message header.

I wonder if the header to the recipient on the generated message isn't forming properly.

Not sure if this is related but spam seems to be released to sender.

Low score spam generates this mail from postmaster[at]fbs.se - clicking the link I see MailScanner release the mail to the origin sender, from apache[at]fbs.se. So it looks as the Spam is released and sent back to the person who sent the mail from the beginning.
